Description
Royal Health Group is the South Shore’s premier Skilled Nursing Facility, offering Short Term Rehabilitation and Medical Management Programs, a specialized Russian Program and Dementia Care.

We are looking for a Clinical Liaison to add to our growing pre-admissions sales team. We are well positioned in various “Accountable Care Organizations” and have dedicated Specialty Program Directors and Care Transitions Coaches to ensure our discharge rate to home is highly successful.

The Clinical Liaison is responsible for generating referrals by building relationships with physicians, hospitals, long term care, independent and assisted living facilities and other community resources. The Clinical Liaison interacts with discharge planners, case managers, clients and families at designated facilities, assisting with the initiation of home healthcare services and the discharge planning process. He or she will support business development by participating in community marketing activities and working to establish strong relationships with new and existing referral sources.

Location: Boston market

Education & Experience:

R.N., LPN, BSW or MSW or B.A. in related field with proven new business development record with hospice, long term facility, or home health care agency
2-4 years of varied clinical liaison or marketing experience, preferably including hospice, home health, community nursing, or case management.
Highly organized and able to multi-task and function independently.
Evidence of achieving referral goals within the market.

The successful candidate will be:

Motivated to integrating into a defined hospital and physician territory on the South Shore.
A goal setter with a proven track record of meeting key business development initiatives.
The ability to clinically evaluate patients for admission into specialty programs, long term care, dementia care, etc.
The ability to bond and build rapport with colleagues and facility based team.
