Core Job Responsibilities
  • Receives and retains referrals from hospital discharge planners, social workers, physicians, or family members.
  • Works with the Clinical Liaison on pre-admission information, obtaining all required financial data, pertinent medical and social information, attending physician information, and other pertinent and required documentation prior to admission.
  • Discusses with Clinical Liaison potential admissions and reviews pre-admission information with Director Nursing Services and others as necessary to determine appropriate placement and eligibility for admission.
  • Reviews with the resident and or/responsible party all post-admission related paperwork including Medicare Part A coverage and non-coverage procedures obtaining signatures as required.
  • Maintains all on-medical record related paperwork in an admissions file for each resident.
  • Provides Health Center tours and pertinent information regarding admissions.
  • Maintains a current wait list for admissions.
  • Attends monthly Resident Services meetings to remain informed regarding the health status of apartment residents and their need for Health Center admission.
  • Maintains complete confidentiality regarding admission information and resident records.
Key Educational/Professional Requirements
  • High school diploma or equivalent;
  •  At least one year previous clerical experience, preferably in a long-term care setting;
  •  Good verbal and written communication skills;
  •  Must be able to maintain confidentiality regarding employee, resident and company proprietary information;
