- Provide care to geriatric patients on a sub acute floor.
- Educate patients and promoting awareness of treatments that would avoid s/he to go back to hospitals
- Assess and evaluate patients’ health status upon admission and throughout duration of stay
- Providing patient and family education and teaching on a variety of health topics
- Communicating effectively with physician, family and staff.
- Provide orientation and training to existing staff
- Keep all paperwork up to date (hard copies and system)
- Assist DON and ED with care-plan planning
- Other duties assigned by the DON or ED
Key Educational/Professional Requirements
- Graduated of an Accredited School of Nursing
- Current and in Good standing License as an RN in the State of Rhode Island
- 2-3 years experience in a Long Term Facility as an LPN/RN (Preferred)
- Ability to work on a fast paced environment
